In 1994, the FDA authorized the use of Human Growth Hormone (HGH)for adults. TESTOSTERONE


Both Men and Women need testosterone for optimal health. Testosterone is needed for libido, muscle strength, leanness, and sense of well being.

Menopausal women have low to non-detectable testosterone levels with resultant loss of libido and increase in body fat. Return of libido and improved mood return within days of testosterone replacement by transdermal cream. Regrowth of muscle and bone occurs within months and is facilitated by a weight resistance training program.

The testosterone level in males peaks at age 17 and declines 1% per year thereafter. A middle-aged male can therefore have a testosterone level 40% below his peak in young adulthood. This problem can be compounded by the increased production of sex binding globulins, which make the lowered testosterone that is present even less effective. Loss of testosterone in males leads to atherosclerosis and heart attacks, obesity, fatigue, depression, and anxiety. These symptoms come on long before the male loses his sex drive or becomes impotent.

There is no evidence that testosterone replacement therapy in males causes prostate cancer or prostate enlargement. In fact, some Urologist-Oncologists say that testosterone may decrease the incidence of prostatic hypertrophy and prostate cancer. ESTROGEN


Estrogen is an essential sex hormone that declines abruptly in women during their menopause. When women do not initiate a prompt estrogen replacement program at the time of menopause, they undergo accelerated aging manifested by: osteoporosis, senility, heart disease, loss of muscle mass, loss of skin elasticity and thickness, atrophy of the female genitalia, and depression.

Estrogen replacement therapy significantly delays or prevents this accelerated aging process seen in women following menopause. In fact, women who take estrogen replacement have half the degenerative diseases of women who do not take estrogen replacement and live an average of 3.5 years longer. What is even more amazing, is that these positive statistics are with those patients on Premarin (non-human estrogen taken from the urine of pregnant horses). We recommend only natural estrogen replacement in the form of topical creams.Then measure estradiol levels and adjusts dosages to pre-menopausal levels We have every reason to believe that the anti-aging effects of natural human estrogen will be even better than with horse estrogen.

A new alternative is Evista (Raloxifen, Lilly). This non-estrogen product protects the heart, bones, and brain just like estrogen but does not have the risk of increases in breast or cervical cancer. In fact, it appears to protect against breast cancer just as well as it's sister Tamoxifen. It should be enhanced with herbal therapy added to prevent hot flashes.

PROGESTERONE


Progesterone is also a female sex hormone that keeps estrogen in balance and is therefore absolutely necessary in the female hormone replacement program regardless of whether the female has a uterus or not. This concept is not understood by some physicians and this has lead to an increase in cancers of the uterus and breast in women on estrogen replacement, alone. Progesterone has powerful anti-cancer properties. In addition, Progesterone facilitates the action of other hormones such as thyroid, testosterone, and endorphins. Synthetic progesterones (progestins) such as Provera lack many of the beneficial properties of Progesterone and in fact actually serve to increase LDL "the bad form of cholesterol". Provera also does not have the beneficial effect that Progesterone has in increasing bone mass, reversing osteoporosis, and burning fat. We recommend only natural progresterone replacement in our female HRT program as indicated by the individual's blood levels.

PREGNENOLONE: "THE SMART HORMONE"


Pregnenalone is a natural hormone made from cholesterol in the body. It is synthesized inside the mitochondria, the tiny "power plants" (up to 1000's per cell) found in each cell. Prenenolone is the basic precursor for the production of ALL the human steroid hormones, including DHEA, Progesterone, Estrogen, Testosterone, Cortisone, Cortisol and Aldosterone.

Pregnenolone is not itself a steroid hormone. Pregnenolone has been used experimentally and medically, since the 1940's when it was found to increase productivity, reduce stress, and to be an excellent anti-inflammatory in conditions such as arthritis and allergies.

Pregnenolone is Very Safe Even at High Levels:
Pregnenolone is not widely used in medicine today in spite of its safety, versatility and efficiency. Pregnenolone is radically safer and more versatile than the specific steroid hormones which replaced it! Prenenolone is safe even at 1000 mg. dosages, a claim no steroid hormone can make. Many women prefer supplementing Pregnenolone because they experience few side effects. Pregnenolone readily converts to progesterone.

Pregnenolone Improves Memory in Animal Studies:
Several animal experiments reveal important clues to Pregnenolone's beneficial effect on memory and learning ability. Pregnenolone seems to reduce the excess release of, or reduce the negative effects of the adrenal stress steroid hormone Cortisol. Cortisol is toxic at high levels. Both acute and chronic stress release of excess Cortisol can damage the hippocampus, a mid-brain region essential to memory formation and retrieval. The mice experiment strongly suggests that Pregnenolone is able to blunt the normal memory damaging effects of excess stress related Cortisol.

Pregnenolone Reduces Allergic Reactions, Lessens Arthritis Inflammation, Improves Mood and Energy:
Pregnenolone shows many of the benefits of Cortisol, it reduces allergic reactions, lessens arthritis inflammation and produces a relaxing and mildly euphoric "stress buffer effect", without any of the negative side effects of Cortisol. Pregnenolone improves mood, and has a mild anti-depressant effect at doses of 50 to 200 mg. per day, and also improves energy levels by protecting our energy producing mitochondria from toxins, which would otherwise damage the mitochondria.

Pregnenolone Acts as a Detoxicant:
Pregnenolone also enhances the activity of detoxifying enzymes, which help our cells (especially the liver and brain) to detoxify various poisons, whether environmental, or our own metabllically produced toxins.

Body's Productions of Pregnenolone Slows as We Get Older:
Pregnenolone is generally safe and effective at doses of 25 to 200 mg. per day. The body's own production of Pregnenolone is reduced with aging, stress, depression, hypothyroidism and toxin exposure. Pregnenolone may be a general "anti-stress" metabolite, which, however, may not always be present in our bodies at optimal levels, precisely because it may be used up in producing all the steroid hormones, not leaving enough to fulfill its stress buffer role. Hence the need for supplementation.

There are actually few side effects as long as the Pregnenalone is of high quality, and the dosage is kept in a safe range. If a poor quality Pregnenalone is used or the dosage is too high problems may result.   • Start by taking 30-60 mg per day of Pregnenolone. Best if taken in the morning before eating.

DHEA


I believe DHEA is an important hormone that we find decreases as we age. Studies also find DHEA to be low in patients with heart attack, diabetes, and osteoporosis. It is believed that by restoring the body's DHEA to youthful levels, you can improve your muscular endurance, increase your energy levels, and have stronger bones. Most people report a having a better "overall sense of well being" after taking DHEA. Recent research in the prestigious New England Journal of Medicine (9/30/99) confirms the safety and effectiveness of DHEA. Both Dr. Hertzberg and his wife Rosalyn take DHEA everyday.

Who Should Avoid DHEA:
Any patient with an active tumor (cancer) should probably avoid taking. Also, any man with an elevated PSA or actual Prostate cancer should probably avoid taking. Finally, women with breast cancer that started after menopause should avoid taking. Women with breast cancers that started pre-menopause are probably free to take but more studies are needed. Some studies show an overall improvement when these women take DHEA, but if you have had breast cancer please discuss personally with Dr. Hertzberg.

Who Should Take DHEA:
Everyone else with a low blood level should consider taking this hormone.

What are the Side Effects?
I believe there are actually few side effects as long as the DHEA is of high quality, and the dosage is kept in a safe range. If a poor quality DHEA is used or the dosage is too high, liver or even heart problems may result. Otherwise acne or breast tenderness may occur. These occur rarely and can be controlled with dose adjustment and/or by using DHEA creams. I expect there will be reports in the newspapers and on TV warning people about these potential problems. Remember that if the DHEA is of high quality and the blood levels are kept properly under control, side effects are rare.

Where To Find DHEA:
DHEA is not a prescription item. It is widely available at drug stores, health food stores, even Sam's Club. Unfortunately, I cannot recommend buying any of these products. The quality is too variable and I am not sure any of these products are safe. Even if your local pharmacist tells you his DHEA is high quality, I still am not sure. Ask the pharmacist if he individually makes these capsules by hand. I believe the only way to be sure the hormone you take is of proper quality is to go to the source. In our office we have available individually hand made by a pharmacist, 100% pure, DHEA capsules. These are micronized, slow release capsules that mimic the exact way DHEA acts in your body. UNLESS YOU CAN GET CAPSULES EQUAL TO THESE IN QUALITY, DO NOT TAKE DHEA.

How Do I Know How Much To Take:
Just as taking any hormone replacement (like Thyroid hormone), blood levels are the key. If your first blood level is low, we will recommend a starting dose. You then are expected to have a blood reading taken about 3-4 weeks later before 11:30 a.m. Further levels will be taken periodically. Those people on TV with liver and heart problems probably never had a blood test taken. They just went to the local pharmacy or health food store and bought some unknown quality DHEA product on their own. You cannot, unfortunately, trust your pharmacist or health store employee. If the product is on a shelf, DO NOT BUY IT.

THYROID


Thyroid hormone is essential for life. Proper thyroid levels are needed for optimal functioning. Conventional medicine replaces thyroid only when indirect laboratory measurements indicate that it is low. We perform more elaborate thyroid testing including measuring the free thyroid hormone, which is the active hormone available to the cells in our body. In addition, we also carefully measure the basal body temperature which is the ultimate indicator of whether we are hyper or hypothyroid. Many times, replacement of human growth hormone or testosterone to more youthful levels creates a greater need for thyroid hormone. In these cases, we add thyroid hormone replacement to our regimen.

BASAL BODY TEMPERATURES

Melatonin is produced by the pineal gland and is stimulated by darkness. This is why we have a pulse of melatonin release several hours after sundown.
